• Nurse Care Coordinator (RN)

    The Nurse Care Coordinator (RN) plays a vital role in advancing high-quality, patient-centered care within our Federally Qualified Health Center (FQHC). They will utilize care coordination, case management and resource management with the purpose of improving the patient’s health, reducing healthcare costs and improving the patient’s healthcare experience. They will collaborate with the patient, the patients primary care physician and other care team members including outside entities and community health partners to help the patient achieve their healthcare goals. Their work may encompass a focus on medical, mental health, substance use and social determinants of health.  

    Key Responsibilities

    • Support team-based care delivery using FQHC and Patient-Centered Medical Home (PCMH) principles
    • Maintain a caseload of patients while performing disease management support and health coaching
    • Monday-Friday -days only - no evenings, weekends or holidays

    Requirements
    • Current Vermont RN license in good standing
    • Associate or Bachelor’s degree in Nursing (BSN preferred)
    • Minimum of 2 years of RN experience preferred
    • Experience in primary care, care coordination, case management, population health, or community health strongly preferred
    • Knowledge of social determinants of health and community-based resources
    • Strong communication, organization, and patient advocacy skills
    • Commitment to the mission and values of community health and serving underserved populations
